I assumed the first Q cast starts at 0 stacks and the first autoattack starts at 2 stacks.

I don't like shiv on Cait because she's already a very strong poker. The best item build for DPS output is IE-PD for the early game, any item substitutions should only be if she needs it (BT instead of IE for sustain against high poke comps, for example)
