Depends in part on the person. People with a really high online profile (Morello for example) will often have their summoner names used in daily conversation, potentially as much as or more than their actual name. People with common first names are also a bit more likely to be referred to by online handle as well from what I've seen.
Summoner names also get used quite a bit when we're in the middle of playtesting. If you're playing against someone who's showing up on screen as 'Volty' for example it's pretty natural to refer to them that way when shouting at them over the wall of a cubicle during the game.
Hey, if you check back on this, are there any plans for a one-stop-shop list of all the Rioters: Summoner name, Real name, Job title, Job 'responsibilities/ which team they are part of, maybe education or relevant prior experience ?
It would be a great sense of connection to the Riot staff, give us a solid idea of where to direct questions, gripes and gratitude, and would be a great informational resource for those interested in getting into the business.
Not sure for the company as a whole sorry. I know Morello's keen to update the design staff page on the website at least at some point though (we've slipped behind a bit there, but that's just too much going on, not a lack of desire to continue with it).

It did, however, lead to an interesting moment when i was having a conversation with -the- Scarra and we both responded at the same time.
