People dont understand that swain is more than just his ult healing himself. Hes one of the best pokers in the game at early levels. Tower poking is even easy beacuse of his Dots. A lil healing reduction? no problem

Swain's ult is also a nice counter to Fizz, since the Tidal Trickster 1. has low health and zero sustain and 2. needs to get so close to Swain to do damage.
Fizz is a strong enough champion that I could see him winning, but I believe Swain has the advantage here.

Any decent Fizz will not even attempt to force a mid kill against Swain unless he has jungle help or Swain is bad.
Fizz will roam bottom/top/enemy jungle and pick up a few kills there. In the meantime, Swain continues farming mid.
In the end, Fizz and Swain come out with nearly the same amount of gold, except Swain has double Fizz's farm and seems to think hes superior.
The difference is that Swain's team is losing other lanes due to Fizz's help, and Swain is simply farming alone mid.
Honestly, I fear a fed Fizz more than a fed Swain, simply due to the fact that a fully built Fizz can one shot the enemy ADC, and one combo Swain. Swain can build tanky, but it won't be enough versus a good Fizz who got his team ahead.

And as the above poster mentioned, Q+E takes a lot of mana (at least 140, and this is without using W at all, which would add another 40).
